BOSTON, MASSACHUSETTS — June 2026 — Form Health has promoted Dr. Emily Smith to the role of National Medical Director, recognizing her outstanding contributions to advancing evidence-based obesity medicine and her leadership in helping scale the organization’s nationally recognized care model.
The promotion marks the latest milestone in Dr. Smith’s six-year journey with Form Health, where she has steadily progressed through a series of increasingly influential clinical and leadership roles. Since joining the organization in 2020 as an Obesity Medicine Physician, she has served as Lead Obesity Medicine Physician and most recently Regional Medical Director, helping strengthen clinical excellence, physician engagement, and patient outcomes across the organization.
Reflecting on her promotion, Dr. Smith shared her continued belief in Form Health’s mission of bringing the highest quality, evidence-based obesity medical care to as many people as possible. She noted that throughout her tenure, the organization has remained committed to delivering care with integrity, compassion, and an unwavering focus on patients.
Before joining Form Health, Dr. Smith served as Director of Medical Obesity at Jesse Brown VA Medical Center, where she led obesity medicine programs focused on improving patient health outcomes through science-based treatment approaches. Concurrently, she served as a Physician and Assistant Professor at UI Health, contributing to both clinical practice and medical education.
Dr. Smith built her clinical expertise through a Fellowship in Endocrinology, Diabetes and Metabolism at Northwestern University, following completion of her Internal Medicine Residency at the same institution. She earned her Doctor of Medicine (MD) from Loyola University Chicago Stritch School of Medicine, establishing the strong academic and clinical foundation that has shaped her career in metabolic health and obesity medicine.
Throughout her career, Dr. Smith has been a passionate advocate for evidence-based obesity care and has played a significant role in advancing awareness of obesity as a chronic medical condition requiring comprehensive, personalized treatment. Her leadership has helped Form Health continue to expand access to high-quality obesity and cardiometabolic care across the United States.
In her new role as National Medical Director, Dr. Smith will oversee national clinical strategy, physician leadership, care quality, treatment innovation, and the continued development of Form Health’s integrated model of obesity and cardiometabolic care.
